Registration Forms: Save Time

Download and complete these forms to bring in on your first visit, or arrive 15 minutes early and complete them when you arrive. Our administrative staff will review your insurance benefits and answer any questions you have.Please print these out and bring them to your first visit:

Initial Evaluation

Our physical therapists will start with a history of your injury and evaluate your condition. With input from you and your physician, they will develop a treatment plan. Your first visit usually includes your first physical therapy treatment.

Treatment Plan

Your physical therapist will also establish an individualized rehabilitation program based on their findings. This plan will include the number of times per week and number of weeks you will need to visit. Home exercises, physical therapy education, and physical therapy goals will be addressed as well. Your treatment plan will be modified as you progress.

What to Bring

  • Medical insurance cards
  • Driver’s license/photo identification
  • List of medications
  • Diagnostic test results (if any)
  • Physician referrals including name, address and phone number of referring physician
  • Calendar to arrange for subsequent treatments, if necessary

Financial Responsibility

At the time of your visit you may be financially responsible for one of the following:

  • Deductible
  • Co-payment
  • Co-insurance
  • Over-the-counter expenses that are not reimbursable through your insurance, such as heating pads or tens unit electrodes

What to Wear

Gowns will be provided or you can wear comfortable clothing in order to allow for the most beneficial therapy. If you have a lower body injury, please wear or bring shorts. If you have an upper body injury, a tank top or camisole (women) would be beneficial.
